The primary purpose of your job position is to plan, develop, organize, implement, evaluate,and direct the Social Work Department in accordance with current existing federal, state, andlocal standards, as well as our established policies and procedures, to assure that the medicallyrelated emotional and social needs of the residents are met/maintained on an individual basis. These responsibilities may include a combination of admissions, documentation, and servicedelivery duties.


Listed below is an outline of the duties and responsibilities that you will be required to perform. As SOCIAL WORKER, you are delegated the authority, responsibility, and accountability to carry out your assigned duties.

  • Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility
  • Meet with administration, medical, nursing staff, and other related departments in planning social services
  • Develop and maintain a good working rapport with interdepartmental personnel and outside community health, welfare, and social agencies to assure that social service programs can be properly maintained to meet the needs of the residents
  • Keep abreast of current federal and state regulations as well as professional standards and make recommendations on changes in policy and procedures to the corporate consultant
  • Serve on various committees of the facility as may be appointed by the Administrator
  • Utilize social work skills in preparation of residents for admission through thorough inquiry and pre-admission conference with the resident and/or family
  • Compile and dissimilate information between the referral source and facility staff pertinent to the resident’s condition and needs to effect admission in a timely manner
  • Evaluate resident needs on admission through the completion of a social history, assessment, and discharge plan
  • Implement the individual resident’s social work care plan involving resident/family
  • Provide inservice training to staff members in relation to resident’s rights and any identifying training needs in relation to the social and emotional needs of the residents
  • Participate in community planning related to the interests of the facility and the services and needs of the residents
  • Interview residents/families as necessary and in a private setting
  • Perform administrative requirements such as completing necessary forms, reports, etc
  • Assist in arranging transportation to other facilities when necessary
  • Refer resident/family to appropriate social service agencies when the facility does not provide the service needs of the resident
  • Provide information to residents/families as to Medicare/Medicaid and other financial assistance programs available to the resident
  • Inform the resident/family of the resident’s personal and property rights
  • Provide consultation to members of our staff, community agencies, etc. in efforts to solve the needs and problems of the resident through the development of social service programs
  • Attend and participate in your professional associations’ activities and programs
  • Maintain an excellent working relationship with the medical profession and other health related facilities and organizations
  • Coordinate social service activities with other departments in the formulation of resident’s individual interdisciplinary resident care plan
  • Work with the facility’s consultants as necessary and implement recommended change as required
  • Review complaints and grievances and make necessary oral/written reports to the Administrator
  • Recommend to the Administrator the equipment and supply needs of the department
  • Perform charting duties as necessary
  • Report all incidents/accidents immediately
  • Work with emotional problems including assisting resident/family with anxiety and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness, and death; and the need for institutional and specialized care
  • Other duties as deemed necessary and appropriate or as may be directed by the Administrator


  • Works in well-lighted/ventilated work areas, as well as throughout the nursing service areas
  • Sitting, standing, bending, lifting, and moving intermittently during working hours

  • Involved with residents, family members, personnel, visitors, etc. under all conditions/ circumstances

  • Communicates with nursing personnel and other department supervisors

  • Attend and participate in continuing educational programs as directed
